Every Meta account I audit has the same problem.
Not broken tracking. Not bad creative. Not wrong audiences. The problem is subtler and harder to fix: two of three things are working, and nobody can figure out why performance is sliding.
The three things are creative diversity, signal density, and simplified structure. You've heard all of them. Every conference talk, Twitter thread, and LinkedIn post about Meta in 2026 mentions at least one. But almost nobody talks about them as what they actually are: a system where each component depends on the other two to function.
Do all three and Meta's algorithm performs. Do two without the third and performance gets worse, not better. That's the part nobody explains, and it's the reason accounts spending $100K, $200K, even $500K a month still plateau or collapse without warning.

Why These Three, Why Now
Meta's advertising infrastructure went through more changes in the last 18 months than in the previous five years combined.
Andromeda, Meta's AI-powered retrieval engine, replaced the old rule-based system that decided which ads were eligible to compete in the auction. Instead of matching ads to users based on the targeting parameters you set, Andromeda uses deep neural networks to analyze your creative content, predict who will engage with it, and decide which auctions your ads can even enter. Your creative is now doing the work that your targeting used to do. GEM (Generative Ads Model) is Meta's foundation model for ad ranking, trained at the scale of large language models. It learns from both organic and paid interactions across every Meta surface and transfers those learnings to hundreds of downstream models. GEM delivered a 5% conversion increase on Instagram and 3% on Facebook when it rolled out broadly in 2025.
Meta Lattice consolidated the many surface-specific ranking models (Facebook Feed, Instagram Stories, Reels, etc.) into a unified architecture, driving a 12% increase in ads quality and 6% improvement in conversions.
And in March 2026, Meta shifted its delivery system from auction-based placement optimization to outcome-based optimization. The system now predicts downstream conversions and lifetime value, not just clicks. Campaigns below 50 conversions per week per optimization objective saw CPM increases of 15-40% overnight.
Each of these changes reinforced the same three requirements: the algorithm needs diverse creative to find the right users, enough conversion data to learn from, and a structure simple enough to concentrate that data where it matters. These aren't new ideas. What's new is how severely the system punishes you for getting even one of them wrong.
What Creative Diversity Actually Means
Creative diversity is the most misunderstood concept in Meta advertising right now.
Most advertisers think they have it. They don't. Taking one hero image and writing six different headlines is not creative diversity. Under Andromeda, Meta's visual recognition models can identify when images with different text overlays are essentially the same creative. The system groups them together, treats them as one test, and moves on. You think you're testing six ads. Meta sees one.
Real creative diversity means running concepts that are genuinely different from each other: different visual treatments, different angles, different formats, different emotional registers. A problem-solution UGC video, a founder direct-to-camera testimonial, a product demo with text overlay, a lifestyle carousel, and an editorial-style static image are five distinct concepts. Five variations of the same product shot with different backgrounds are one concept wearing five outfits.
The reason this matters is mechanical, not philosophical. Andromeda's retrieval system can process 10,000x more ad variants in parallel than the old system. It's designed to match specific creative concepts to specific user segments based on behavioral signals. When you give it genuinely diverse creative, it can find pockets of demand you didn't know existed. When you give it five versions of the same thing, it finds one audience, saturates it, and your CPMs spike.
Meta's own data shows that after four exposures to the same ad, the probability of conversion drops by roughly 45%. Creative fatigue used to be a problem you dealt with every 6-8 weeks. Under Andromeda, effective ad lifespan has compressed to 2-4 weeks. The system finds your best audience faster, which means it exhausts that audience faster too.
The operational implication is significant. You need a creative pipeline, not a creative calendar. The difference: a calendar plans assets on a schedule. A pipeline produces them continuously, with enough conceptual range that each new batch gives the algorithm something genuinely new to work with. The accounts winning right now produce 15-30 active creatives at any given time and refresh weekly. That sounds like a lot until you realize that Meta's algorithm needs 15-50+ active creatives to optimize properly. Below that threshold, you're leaving performance on the table.
One test from Scaledon showed this directly: a single ad set with 25 diverse creatives produced 17% more conversions at 16% lower cost versus a traditional 5-ad-set structure with fewer creatives in each. The creative volume combined with structural consolidation outperformed the fragmented approach across every metric.
Signal Density: The Math Nobody Does
Signal density is the concept that separates accounts that scale from accounts that stall. It's also the one most advertisers skip because the math is uncomfortable.
The definition is simple: signal density is the volume and consistency of conversion data per ad set. Meta's algorithm needs approximately 50 conversions per week per ad set to exit the learning phase and optimize delivery reliably. Below that threshold, the system doesn't have enough data to build a stable predictive model. It hedges by raising your CPMs as a risk buffer.
The math works like this. Multiply your target CPA by 50. That's the minimum weekly budget each ad set needs to generate enough signal. If your CPA is $50, each ad set needs $2,500 per week, or roughly $357 per day. If your CPA is $100, it's $5,000 per week, or $714 per day.
Now count how many ad sets you're running. If the answer is more than your total weekly budget divided by (CPA × 50), you have a signal density problem. Most accounts do. The typical audit reveals 8-12 ad sets competing for a budget that can only support 2-3 at adequate signal density.
This is where the math gets uncomfortable: if you're spending $10,000 per week and your CPA is $100, you can only support two ad sets before signal starts to dilute. Most advertisers running that budget have five or six. Each one converts maybe 8-10 times per week. The algorithm can't learn from 8 conversions. It guesses. It hedges. Your CPMs inflate. And because each ad set looks like it's performing at a marginal level, nobody kills them. They just slowly drain budget from the ad sets that could actually scale.
After Meta's March 2026 update, this dynamic intensified. The new outcome-based optimization system doesn't just predict whether someone will click or convert. It models the full path from impression through purchase, incorporating post-purchase signals like return rate and lifetime value. Building that model requires more data, not less. Campaigns that were borderline functional at 30 conversions per week are now definitively underperforming.
There's a secondary signal density layer that most advertisers miss entirely: event match quality. This measures how accurately Meta can match your conversion data back to specific users. It's scored on a 1-10 scale in Events Manager. Data from multiple sources shows that improving your Event Match Quality score from roughly 8.6 to 9.3 can reduce CPA by approximately 18%. That's not a creative improvement or a targeting improvement. That's pure plumbing. Signal density isn't about spending more. It's about spending in fewer places so each dollar teaches the algorithm something useful.
Simplified Structure: The One Nobody Wants to Do
Simplified structure is the hardest of the three because it requires killing things that look like they're working.
The old Meta playbook was built on segmentation. Separate campaigns for prospecting and retargeting. Different ad sets for lookalikes, interest stacks, and custom audiences. Segmented by age, by geography, by device. This was sophisticated media buying in 2021. In 2026, it's actively working against you.
Meta's algorithm now handles retargeting automatically within unified campaign structures. Advantage+ campaigns typically allocate 20-30% of budget to engaged audiences and past customers without you setting up a separate campaign for it. The dedicated retargeting campaign you've been running for three years isn't just redundant. It's competing with your prospecting campaigns in the same auctions, fragmenting your conversion signal, and preventing both campaigns from optimizing properly.
The recommended structure for most accounts in 2026 is two campaigns. One for testing new creative concepts at modest budget. One for scaling winners with the majority of spend. That's it. Some accounts add a catalog campaign for DPA if they have large product catalogs, or a separate campaign for major seasonal events. But the default is two, not ten.
This feels reckless if you've spent years building elaborate campaign structures. It feels like surrendering control. In a sense, it is. You're surrendering control of audience selection and bid optimization to an algorithm that, in 2026, is meaningfully better at those tasks than you are. What you keep control of is creative strategy, conversion architecture, and budget allocation across campaigns. The data supports this. Consolidated accounts consistently outperform fragmented ones because every conversion feeds back into a larger data pool that the algorithm can learn from faster. One campaign at $500/day outperforms five campaigns at $100/day each, even if the total spend is identical. The reason is data liquidity: the consolidated campaign builds a stable predictive model in days, while the fragmented campaigns never exit the learning phase.
Simplification also interacts directly with creative diversity. When you have 25 creatives in one ad set instead of 5 creatives in each of 5 ad sets, the algorithm can test creative-to-audience matches across the full audience pool. In a fragmented structure, a creative that would have performed well with a different audience segment never gets the chance because it's locked into one ad set's limited reach.
The Loop
These three components create a reinforcing loop, and understanding the loop is what separates operators who understand Meta from those who are just following a checklist.
Simplified structure concentrates budget into fewer campaigns and ad sets. That concentrated budget generates signal density by pushing each ad set above the 50-conversion-per-week threshold. With sufficient signal, the algorithm can reliably sort through a diverse creative library to find which concepts resonate with which user segments. The creative diversity then expands reach by finding new audience pockets, which generates more conversion data, which strengthens signal density further.
Pull one out and the loop breaks. Not gradually. Mechanically.
Creative diversity without signal density is noise. You've got 30 creatives, but each one gets seen by too few people to generate meaningful data. The algorithm can't distinguish between a bad creative and a good creative that just hasn't had enough exposure yet. Everything looks mediocre. You kill creatives that would have worked and keep ones that won by luck.
Signal density without creative diversity is a plateau. You've consolidated budget beautifully. Each ad set has 80 conversions per week. But you're running four variations of the same concept. The algorithm finds one audience fast, saturates it within two weeks, and your CPMs start climbing. You have plenty of data, but the data is all telling you the same thing about the same audience.
Simplified structure without creative diversity or signal density is an empty container. You've got two clean campaigns and broad targeting. But if the creatives are too similar, or the budget per ad set is too thin, consolidation alone doesn't help. You've just built a more organized version of the same problem.
Diagnosing Your Account
Before you change anything, diagnose which leg of the system is broken. Applying the wrong fix accelerates the problem.
Signal density audit. Pull your weekly conversion count per ad set for the last 30 days. If any ad set is below 50, it's operating on insufficient signal. Count how many ad sets are above the threshold. Then do the budget math: divide your total weekly budget by (CPA × 50) to get the maximum number of ad sets your budget can support at adequate signal density. If you're running more ad sets than that number, consolidation is your first move. An account spending $15,000 per week with a $75 CPA can support exactly four ad sets. If you're running eight, you're splitting signal in half across every one of them and probably wondering why nothing exits learning.
If your conversion volume is genuinely too low to hit 50 per week on your primary event, move up the funnel. Optimize for Add to Cart or Initiate Checkout instead of Purchase. The algorithm needs volume to learn. A high-volume mid-funnel event that reliably predicts purchases will outperform a low-volume bottom-funnel event that starves the model of data.
Creative diversity audit. Look at your active creatives. Ignore the count and look at the concepts. Group them by visual approach and messaging angle. If more than half fall into the same conceptual bucket, you have a diversity problem regardless of how many total ads you're running. Five product-on-white statics with different headlines is one concept, not five. A problem-solution UGC video, a lifestyle carousel, a founder testimonial, a text-heavy editorial static, and a product demo are five genuinely different concepts. Check your Creative Similarity score in Ads Manager if available. Check frequency: if your 7-day frequency exceeds 3, creative fatigue is compounding whatever else is wrong in the account.
Structure audit. Count your active campaigns and ad sets. Look for audience overlap between ad sets. Pull the audience overlap tool and check whether your "prospecting" and "retargeting" campaigns are competing for the same users in the same auctions. Look for retargeting campaigns that duplicate what Advantage+ is already doing automatically. Check whether any campaign is a legacy holdover that exists because "it's always been there" rather than because it serves a distinct strategic purpose. If you find a campaign that's been running since 2023 with 12 ad sets and a combined 40 conversions per week, that's not a campaign. That's a budget leak with a dashboard attached.
Event match quality audit. This one gets overlooked because it lives in Events Manager, not Ads Manager. Navigate to Events Manager, select your data source, and check your Event Match Quality score. If it's below 8, you have a data plumbing problem that no amount of creative or structural improvement will fix. The algorithm is making optimization decisions on incomplete data. Implement server-side tracking through the Conversions API if you haven't already, deduplicate events between Pixel and CAPI, and pass as many customer information parameters as possible. The gap between an EMQ of 7 and an EMQ of 9 can represent roughly an 18% CPA reduction with zero changes to creative or structure.
Fix signal density first. It's the foundation. Without enough data per ad set, you can't accurately evaluate creative performance, and structural changes won't stick because the algorithm can't optimize with insufficient information.
Then simplify structure. This usually means consolidating ad sets and campaigns, which simultaneously improves signal density.
Then invest in creative diversity. This is last not because it's least important, but because the impact of diverse creative is invisible when signal density is too low or structure is too fragmented for the algorithm to act on what it learns.
The System Is the Strategy
The accounts winning on Meta right now didn't adopt three best practices from a conference talk. They built one system.
Creative diversity keeps the algorithm finding new audiences. Signal density gives it enough data to learn which audiences convert. Simplified structure concentrates that data where it can compound.
This isn't a 2026 tactic. Every infrastructure update Meta has shipped in the last 18 months, and every one they'll ship going forward, rewards these three inputs more aggressively. The algorithm is getting smarter and faster. What it needs from you is getting simpler. It just isn't getting easier.
